What to Bring to your first appointment

1. Copy of Referral

2. Medicare/DVA card

3. Test and Investigation results (for example, blood tests, X-rays, scans)

4. Current medication list or medications

5. Complete Vaccination history (including records of childhood vaccinations, annual flu shots etc.)

We hope you understand that sometimes the specialist needs to respond to urgent situations, which could mean that your appointment may be delayed on the day. Please allow extra time for finding a park when arriving for your appointment
